- Review the Parish Plan Policies, Agreement and Procedures form (Parish Plan Forms)
- Complete and return the Agreement form to CDU, 120 E. Colonial Hwy, Hamilton, VA 20158 or fax to 540.338.4788
The program coordinator is responsible for the enrollment process as listed below. After the enrollment process, participants will work directly with CDU. Once a year, the coordinator will be asked to complete an Online Program Evaluation form and forward this to CDU.
