This form outlines the responsibilities of the parish, parish coordinator, and individuals participating in the parish plan.

Parish Plan Online Program Coordinator:
This form is to be used by the Parish Plan Ccoordinator when sending in group tuition and individual enrollment forms.

Parish Plan Online Program Enrollment:
This form is used by each person enrolling in a seminar or course as part of the Parish Plan. This form is completed by each individual and given to the Parish Plan Coordinator to submit to CDU.
